Forms.VET ("Forms Dot Vet") lets your clients fill out onboarding forms and authorization forms from your website, using their own device. All forms are also printer-friendly for those who prefer pen and paper.

How it works

Forms.VET is simple. Just four steps between today and a calmer lobby:

1) Tell us which forms you need

Send us an example of the forms you need for your website: a link to an existing form, a PDF of a form you created yourself in Microsoft Word, or a scan/picture of a drawing you made of the form you'd like to have.

Our purpose is to make pet clinics and veterinarians' lives easier. That starts with working with what's easiest for you. We can even do this by phone. #SoFlexible

2) We create the forms for you

We design your forms. They'll be mobile-friendly for those who want to fill-out forms on the go, and they'll be printer-friendly for those who prefer pen and paper.

Your forms will look professional and they'll integrate with your website by using your clinic's logo and colors.

Any form you need, we can create:

• New Client Application, for new humans
• New Patient Form, for new pets
• Drop-off Form / Appointment Check-in Form, for existing pets
• Surgical / Dental Consent Form
• Anesthesia Consent Form
• Donation Form, for any pro bono work your clinic does
• Etc. — just tell us what you need

3) Put the code on your website

Once you've reviewed the forms, we'll send you a piece of code to put on your website. That code will generate a clickable button to launch or print each form.

Your clients will select the form they need. Then, they'll fill it out online or print it out and bring it along. Your staff can use those same buttons to print a form.

4) We handle the rest

When a client submits a form from your website, that data is emailed to your office in your preferred format.

By default, you receive the form's content as an email. However, we can make it downloadable in a format that suits your existing workflow. (E.g., as a file you can then import into your practice management software.)

Here's a pet drop-off questionnaire form as viewed from a desktop computer / laptop / tablet and from a smartphone. The online forms will also respect the light theme / dark mode preference on your customers' device.
